Resultatet blir en \u00e5tg\u00e4rd som riskerar att felet \u00e5terkommer eller att individen\/individerna i fr\u00e5ga f\u00e5r b\u00e4ra ansvaret f\u00f6r n\u00e5got som kanske har sin f\u00f6rklaring inom andra kategorier.<\/p><p>Faktum \u00e4r att merparten av de analyser jag sj\u00e4lv varit med om kring m\u00e4nskliga fel har sin riktiga orsak i uppbyggnaden av processen. Det kan vara arbetsplatsens layout, framtagen metodik eller att vi inte skapat r\u00e4tt f\u00f6ruts\u00e4ttningar f\u00f6r de som arbetar i processen.<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/section>\n\t\t\t\t<section class=\"elementor-section elementor-top-section \u00e4r det?<\/h2>\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-ebdb1c7 elementor-widget elementor-widget-text-editor\" data-id=\"ebdb1c7\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t\t\t<p>Om det helt saknas en standard \u00e4r risken stor att arbetet kommer att utf\u00f6ras p\u00e5 olika s\u00e4tt. Uppst\u00e5r det fel i en process som saknar standard b\u00f6r vi rikta fokus mot st\u00f6dfunktionerna och v\u00e4rdefl\u00f6dets ledarskap.<\/p><p>Det \u00e4r st\u00f6dfunktionerna (exempelvis Underh\u00e5ll, Teknik och Kvalitet) som har till uppgift att skapa f\u00f6ruts\u00e4ttningar f\u00f6r v\u00e4rdefl\u00f6det. Det \u00e4r ledaren f\u00f6r fl\u00f6det som ska beg\u00e4ra att f\u00f6ruts\u00e4ttningarna finns och s\u00e4kerst\u00e4lla att personerna i fl\u00f6det har f\u00e5tt r\u00e4tt f\u00f6ruts\u00e4ttningar innan de p\u00e5b\u00f6rjar sitt arbete.<\/p><p>Om standard finns kan det vara s\u00e5 att personen i fr\u00e5ga inte f\u00f6ljt standarden men orsaken till detta kan vara flera. Standarden kanske inte \u00e4r tillr\u00e4ckligt tydlig eller s\u00e5 \u00e4r den bristf\u00e4lligt uppbyggd. Kanske \u00e4r milj\u00f6n runt arbetsplatsen fel och skapar risker som framtagen standard inte tagit h\u00e4nsyn till.<\/p><p>Det kan \u00e4ven vara s\u00e5 att personen i fr\u00e5ga \u00e4r flyttad fr\u00e5n en avdelning till en annan utan att ha f\u00e5tt f\u00f6ruts\u00e4ttningar att s\u00e4tta sig in i aktuell standard.<\/p><p>Som du ser \u00e4r det om\u00f6jligt att direkt avg\u00f6ra vems felet \u00e4r och f\u00f6r att vara helt tydliga \u00e4r det inte det vi s\u00f6ker efter. Vi s\u00f6ker orsaken till att det m\u00e4nskliga felet uppstod f\u00f6r att kunna avhj\u00e4lpa orsaken fram\u00e5t. Syftet med en standardisering \u00e4r att vi skapar det b\u00e4sta t\u00e4nkbara arbetss\u00e4ttet utifr\u00e5n den data och erfarenhet vi har just nu.<\/p><p><span style=\"font-size: 15px;\">Denna standard kan och b\u00f6r sedan utmanas l\u00f6pande med nya f\u00f6rslag. Efter f\u00f6rsta tempot (bearbetningen) ska produkten tv\u00e4ttas med medel 123 och sedan placeras i pall enligt framtagen instruktion.<\/p><p><span style=\"font-size: 15px;\">Om vi fr\u00e5ng\u00e5r standarden ovan och exempelvis \u00e4ndrar sk\u00e4rhastigheten i maskinen uppst\u00e5r en risk. Det \u00e4r en metodik som ger oss m\u00f6jligheten att b\u00e4ttre f\u00f6rst\u00e5 orsaker bakom m\u00e4nskliga fel och d\u00e4rmed \u00f6ka m\u00f6jligheterna att f\u00f6ra in en l\u00f6sning som tar bort risken att det m\u00e4nskliga fel uppst\u00e5r igen.<\/p><p><span style=\"font-size: 15px;\">Vanligt f\u00f6rekommande svar i kundreklamationer kopplade till just m\u00e4nskliga fel \u00e4r:<\/span><\/p><ul><li>\u201dVi har utbildat personalen\u201d<\/li><li>\u201dVi har g\u00e5tt igenom rutinerna med ber\u00f6rd personal\u201d<\/li><\/ul><p>Eller \u00e4nnu v\u00e4rre:<\/p><ul><li>\u201dBer\u00f6rd person arbetar inte l\u00e4ngre kvar i verksamheten\u201d.<\/li><\/ul><p>Samtliga dessa svar indikerar att vi brister i rotorsaksanalysen och att medarbetarna i fl\u00f6det f\u00e5r b\u00e4ra ansvaret i alla l\u00e4gen. Min erfarenhet \u00e4r att personalen i de operativa delarna ser positivt p\u00e5 verktyget eftersom man i de flesta fall hittar orsaken till problemet i uppbyggnaden av processen. Det \u00e4r inte ovanligt att det framkommer information om att personalen vid flertalet tillf\u00e4llen redan p\u00e5pekat bristerna men att aktiviteter inte utf\u00f6rts.<\/p><p>Inf\u00f6randet \u00e4r dock av stor vikt och kr\u00e4ver planering f\u00f6r att lyckas. Har vi inte f\u00f6rsett individen med r\u00e4tt f\u00f6ruts\u00e4ttningar blir den direkta \u00e5tg\u00e4rden att t\u00e4ppa till dessa brister.<\/li><li>Den andra sektionen bygger p\u00e5 fr\u00e5gor kopplade till 4st kategorier:<\/li><\/ol><ul><li>Tekniska och\/eller process-problem<\/li><li>Milj\u00f6f\u00f6rh\u00e5llande<\/li><li>Ouppm\u00e4rksamhet \/ F\u00f6rbiseende<\/li><li>Attityd och beteende<\/li><\/ul><p>Totalt s\u00e4tt finns det 20st fr\u00e5gor inom dessa kategorier och det \u00e4r bland dessa fr\u00e5gor som vi kommer att hitta t\u00e4nkbara orsaker. Analysen bygger p\u00e5 dialog mellan chef och ber\u00f6rd individ. Utfallet fr\u00e5n dialogen verifieras l\u00f6pande med fakta fr\u00e5n processen.<\/p><ol start=\"3\"><li>Sektion 3 visar f\u00f6rslag p\u00e5 t\u00e4nkbara aktiviteter f\u00f6r att hantera de brister man fastst\u00e4llt. Det kan exempelvis handla om instruktioner, visuella st\u00f6d eller f\u00f6r\u00e4ndringar av arbetsplats-layout.<\/li><li>Den sista sektionen visar vilka aktiviteter som ska f\u00f6ras in, av vem samt n\u00e4r. Resultatet fr\u00e5n denna analys l\u00e4mnas sedan tillbaka till arbetsgruppen (reklamationsgruppen) som kan forts\u00e4tta med implementeringen samt verifieringen.<\/li><\/ol><p>Summerat \u00e4r detta ett kompakt och enkelt verktyg som skapar b\u00e4ttre m\u00f6jligheter att hitta orsaken bakom det m\u00e4nskliga felet.
